Mission

At Clear Path for Veterans New England, we empower Veterans and Service Members adjusting to civilian life after the military to develop a life action plan that optimizes their health and wellness by enabling them to reach their full potential. SSVF Case Managers will do this through the assignment of Clear Path Programming in conjunction with our Outreach Team. We deploy our Peers, Case Managers and Wingmen within their communities to conduct outreach and find Veterans or Service Members in need. Whether it is dealing with a veteran who is homeless or food insecure, finding a veteran a job, or dealing with an immediate need, Peers, Case Managers, and Wingmen are there to encourage Veterans and Service Members that they meet in their communities to reach out for help and will refer them to the Clear Path for Veterans New England team to work with Peer Mentors and case managers to develop a life action plan. Clear Path works hand in hand with the Veteran or Servicemember by supporting progress against action plans to identify if additional help or resources are needed.

Overview

Clear Path SSVF Case Managers serve as employees of Clear Path for Veterans New England. As representatives of the organization, they reflect the values, goals, and mission of Clear Path for Veterans New England.
This position provides SSVF Case Management services to Veteran households to develop and achieve individual stabilization plans. This role will operate across Massachusetts and New Hampshire and meet with Veterans in the office and the field.

Roles and Responsibilities

You will perform the following tasks as a Clear Path for Veterans New England SSVF Case Manager.
Provide Housing Stabilization services including but not limited to:

Case Management
Develop assessments of client’s needs.
Develop a Housing Stability Plan that will ensure the stability of household needs.
Provide clients with VA state and local benefits.
Administer and provide Temporary Financial Assistance as needed.
Provide clients with referrals for services.
Provide follow-up services to clients.
Maintain an organized and accurate client file system.
Provide data and narrative information for periodic reports.
All other duties assigned.
Ability to navigate a client tracking system and maintain current data and reporting.
Consults with program applicants to assess need and determine program eligibility, ensuring that clients obtain needed supportive services through an intake assessment.
Provides ongoing support and expertise through assessment, planning implementation, and overall evaluation of the Veterans' needs as drawn from his/her goal/action plans.
Obtains information contributing to the client’s situation and evaluates these, along with the client’s abilities and skills.

Qualifications

Requirements

Veteran preferred but not required.
If a veteran or non-veteran is hired, they must be able to share your experience and information with Service Members and Veterans throughout your community.
Conduct outreach to present yourself as an SSVF Case Manager within your assigned State / Community COC or program assignment.
Attend a variety of Community engagements along with Clear Path for Veterans New England’s Wingmen/Women and Peer support specialists.
Veterans must provide a DD-214 package.
1-3 years of experience in a leadership role, either military or human service field
Reliable transportation
Valid Driver’s License
Pass CORI
Three personal references

one-year of related work experience, associate or bachelor’s degree preferred but not required.

Hours

Positions available F/T
40 Hour work weeks Monday-Friday, possible weekends if needed to support the mission.
Pay will be determined by experience.
